Project description
Case management has a significant role in the changing the health and social care service system in Finland, which has largely been redefined locally and spatially after the wellbeing counties started their operations in 2023. Thus, this project investigating care management of older adults in care needs is placed in a socially significant phase. The research produces key information on the effects of social services and social work methods and the assessment of effectiveness. Results can be used to support development of client, longitudinal studies, and decision-making.
With the policy recommendation of aging in place, ties to institutional places and spaces have become more diverse. In the future, it will become even more important for older service users to easily find a place where service advice and guidance is available. Successful case management can strengthen the subjective well-being and safety experienced by the older adults and increase the effectiveness of the services. Confidence in being able to manage in one's own home and living environment increases client safety. At the same time, ending up at the "wrong" services and service locations decreases, which increases customer satisfaction and cost efficiency.
